MEDIA RELEASE: I grant permission to Triskelion Arts to take and use photographs AND/OR video of me taken at 106 Calyer Street in publications, online, and in other communications related to the mission of Triskelion Arts

By participating in Triskelion Arts' program, I hereby agree to the following: I represent that I am 18 years of age or older and legally capable of entering into this agreement. I further agree to cooperate and conform with the directions, policies, rules, and instructions of Triskelion Arts personnel and its staff with respect to any activity during this program. I am not aware of any medical condition which would render it inappropriate for me to participate in this program and related activities; and I assume the risk of any unknown medical condition I may have that might be affected by this program activities or related actions. I understand that Triskelion Arts and its staff do not have medical personnel available. I further understand that Triskelion Arts and its staff assume no responsibility for any injury or damage that might arise with such treatment. As consideration for my being permitted to participate in this program, I agree to forever release, discharge, and hold harmless from any legal and/or other liability and agree not sure Triskelion Arts and its teaching staff, employees, agents, representatives, members, shareholders, officers, directors, coordinators, parents, volunteers, assigns, heirs, next of kin, and/or estates (hereafter collectively the “Organization”). I understand that my participation in this program involves the risk of injury, including, but not limited to, falls, loss of control, collisions, accidents, and physical and/or psychological injuries, and I agree to assume all risks and all liabilities connected with the above-described class activities. I hereby agree to indemnify the Organization from any and all liability, loss, costs, claims, fees, judgments, or damages the Organization may suffer arising out of, or related to, or on the behalf, or because of my actions and inactions, or by a third party, or as a result of, the negligence or default of the Organization. I further agree this Release shall be construed in accordance with the laws of the State of New York. If any term or provision of the Release shall be held illegal, unenforceable, or in conflict with any law or stature governing this Release, the validity of the remaining portions shall not be affected thereby. I understand and agree that this Release and Waiver of Liability form relates to and bind the undersigned, including my family, heirs, assigns, agents, legal representatives, administrators, trustees, estate, and any other interested person(s) or entity. I have read the Release fully, understand its terms, and understand that I have given up substantial rights by signing it and have signed it freely and voluntarily. I further intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law. I sign this Release voluntarily as my own free act and deed. I execute this Release for full adequate and complete consideration, fully intending to be bound by the same. This release is NOT NEGOTIABLE and exists to protect the livelihood of the Organization, so it can continue serving artists like you. Thank you!
